You're confirmed for your 2026 Medicare Plan Review.

We will review plans available in your area and help you choose coverage based on your specific needs, medications, and preferred pharmacy.

Please have the following available:

1. MyMedicare User ID and Password

This login will allow you to easily access your most recent medication list and help us if we need to make changes to your 2026 coverage. Please review your medications for accuracy prior to our call. 

IMPORTANT: If you’re not sure if you have a MyMedicare login, or you need to reset your password, please visit www.mymedicare.gov prior to your phone meeting. 

2. Original Medicare ID Card (red, white, and blue ID card)

3. Medication List

If you cannot access your MyMedicare login info, you can provide us with a list of your medications through the following link. 

Click here to submit your medication list (this is not necessary if you have your MyMedicare login info)

4. Computer and Internet Access

It is not required, but please try and have access to a computer or tablet.  We do not recommend that you use a smartphone.
